There are no changes in the information requested for the census this year. However, please remember that due to changes made in 2009 to federal requirements for reporting vesting, census data must include ACTUAL NUMBER OF HOURS WORKED for each employee.
We can no longer accept estimates of more or less than 1000 hours.
For full-time, salaried employees, you can enter 2080 hours worked.
For part-time or terminated salaried employees, please estimate the number of hours according to the employee's typical work schedule.
Also please note that first name, last name, and middle initial are divided into separate fields on this new template. Middle initials should consist of only one letter.
Instructions for download
Click on the link above to download the current census. Depending on your browser, one of the following may occur:
The file opens within your browser window or within Microsoft Excel: You may enter the information directly on this screen. To save your changes or to save a copy of the file on your system, click on "File" in the menu bar, select "Save As" and choose where to save the file on your system. You must save the changes on your system in order to submit the file.
The browser prompts you to either open the file or save it to disk: Choose "Open" or "Open with" (specify Excel if it is not the default). The file will open in Excel. You may enter the information directly on this screen. Then, to save your changes or to save a copy of the file on your system, click on "File" in the menu bar, select "Save As" and choose where to save the file on your system.

Instructions for completing the census
We need the information listed on the worksheet for each person who was an employee of your company in 2015, regardless of whether or not that person was a participant in the 401(k) plan.
(Make sure that when you are reporting total gross earning that you report gross wages from the final payroll report.)
All information MUST be provided. If there is missing information or any rework needs to be done an additional charge may be imposed.
Please return the census information to our office as soon as possible after the end of the year but no later than January 31, 2016.

How to require a password to open a file in Microsoft Excel
When you create a password, write it down and keep it in a secure place. If you lose the password, you cannot open or gain access to the data in a password-protected workbook.
- On the File menu, click Save As.
- On the Tools menu, click General Options.
- In the Password to open box, type a password, and then click OK.
Passwords are case sensitive. Type the password exactly as you want users to enter it, including uppercase and lowercase letters.
- In the Reenter password to proceed box, type the password again, and then click OK.
- Click Save.
- If prompted, click Yes to replace the existing workbook with the open workbook.
